Training Records Request – Phoenix East Aviation: Forms

Students, when you complete your Flight Training at PEA or if would like to request a copy of your student Training Records (which includes a copy of each of your Graduation Certificates and a letter from the Chief Pilot) please complete the Training Records Request from below*.

*Please note that Flight Training Record Requests may take 2-3 business days to process, plan accordingly. You will be notified when your records are processed and ready to be picked up.
